Mandate

Approach clients with chronic disease in an all-inclusive manner, addressing the medical and coping mechanisms to address the condition(s).
Provide referrals for relevant services and information to address the needs of the clients’ medical condition(s).
Put into practice shared care model with primary care medicine and specialists.
Respectfully transfer the clients to centres where certain services are required that the NDG medical centre cannot provide.
